Abstract

Objective

To describe the pharmacological characteristics and drug interaction profile of the novel erectile dysfunction drug, Viagra®.

Design

We conducted a parallel design study. The design was based on a randomized, double-blind, placebo-controlled study with 10 men, all of whom received Viagra® for erectile dysfunction. Viagra® was administered orally to each participant with an oral dose of 100 mg, with the following bioavailability: 77.6% after 3 h. Patients were administered a single dose of Viagra® at the beginning of the study and the next 6 h after the next dose.

Study population

The study was carried out between October 2003 and May 2004 in a large tertiary center in Italy. Patients were recruited at the beginning of the study and in the following month, they were enrolled, followed by another study in the same setting.

Inclusion criteria were patients with a diagnosis of erectile dysfunction. Exclusion criteria were patients with a diagnosis of diabetes mellitus, patients with chronic renal failure, patients with a history of liver disease or with history of liver injury, patients with a history of stroke, patients who were taking any of the following: nitrate-based products (e.g., nitroglycerin, isosorbide dinitrate, isosorbide mononitrate, nitrostat, tadalafil, vardenafil), or any of the following, either as an inducer or inhibitor of phosphodiesterase-5 (PDE5) activity: PDE5 inhibitors, as previously described, or any other medication with an active metabolite such as a CYP1A2 inhibitor, or any medication that could be metabolized by CYP1A2, or any other drug that could be metabolized by CYP1A2.

Procedure

The protocol for this study was approved by the ethical committee of the University of Bologna, the Italian Ministry of Health (MOSA-II). All participants underwent a complete assessment of the baseline characteristics and lifestyle habits. All participants were asked to complete an informed consent form, which was administered orally at the beginning of the study. The participants were also asked to stop the Viagra® and the PDE5 inhibitors and the drugs they had been taking for a minimum of 24 h. All participants received a dose of Viagra®, one hour before the PDE5 inhibitors and one hour after the Viagra®.

The study was registered in the Clinical Trials Registry for International studies (CTIR) and the ClinicalTrials.gov registry, respectively. All patients were required to take a single dose of Viagra® for at least 1 h, and the drug was not expected to cause any adverse events.

Intervention

The Viagra® study was carried out in the Italian hospital and was performed at a single site in Italy between December 2004 and February 2005. Patients were randomized to receive a single dose of Viagra® at the beginning of the study and a single dose of a single dose of PDE5 inhibitor, either as an inducer or inhibitor of phosphodiesterase-5 (PDE5) activity, at the same time as their baseline measurements. Patients were asked to stop the PDE5 inhibitor and to wait for the second dose of Viagra®.
